Comments:

A mandated Renewal inspection was conducted today. There were 6 children present (12 points) with 1 caregiver. Reviewed 2 children's files, 2 staff files, emergency plan and emergency evacuation log, attendance, menus, medications and authorizations, training hours and physical plant. Standard #:	22VAC40-111-700-A
Description:	Based on record review, prescription emergency medications did not have written authorization from physician with instructions for dosage, frequency and amount for allergies. Evidence: Child#1 did not have physician authorization or instructions for emergency Epi pen.
Plan of Correction:	I will have the parent's get the authorization and other information.
Standard #:	22VAC40-111-210-A
Description:	Based on record review, caregivers did not complete 16 hours of annual training. Evidence: Caregiver#2 was missing 6 hours of annual training.
Plan of Correction:	5 hours will be completed this weekend and the last hour by 12/24/19
