Technical Assistance:
Removing the plastic bags from crib mattresses was discussed with the provider.

Comments:
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 9:00 AM and departed at 11:00 AM. Four children, totaling eleven points were in care with the provider. Morning activities were observed. Two provider/caregiver records and four children's records were reviewed. The provider stated that no medication has been administered to the children. No children's injuries were reported.

An exit meeting was conducted with the provider prior to closing the inspection.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based upon review of the posted emergency preparedness plan, the provider has not ensured that an annual review and update of the emergency preparedness plan is documented.
Evidence:
The posted emergency preparedness plan indicates a 2017 date and does have documented a more recent review and update, if needed.

Plan of Correction: The provider responded with the following:
The emergency preparedness plan will be reviewed and updated, if necessary. This review will be dated. The plan will be reviewed annually in the future.

Standard #: 22VAC40-111-100-A
Description: Based upon review of four children's records and provider interview, the provider has not ensured that a physical examination under the direction of a physician is obtained prior to attending or within 30 days after the first day of attendance.
Evidence:
1. The record provided for child 1 documents a start date of 9/3/2019. The record did not include documentation of a physical examination.
2. The provider verified that the record did not contain documentation of a physical examination.

Plan of Correction: The provider responded with the following:
The parent was contacted during the inspection and will obtain a copy of the child's well baby check (physical).

Standard #: 22VAC40-111-60-B
Description: Based upon review of four children's records and provider interview, the provider has not ensured that each child's record includes the names, addresses and telephone numbers for two designated persons to be contacted in an emergency when a parent cannot be reached.
Evidence:
1. The record provided for child 1 included documentation required for only one designated emergency contact person.
2. The provider verified that the record included only one designated emergency contact person.

Plan of Correction: The provider responded with the following:
The parent will be asked to provide information for a second designated emergency contact person.
