Technical Assistance:
Discussion was held on the topics of qualification documentation required for the substitute provider and to follow up with the power company to take care of the cables on the backyard area near the playground.

Comments:
An unannounced monitoring visit was conducted today between the hours of approximately 8:45 am through 1 pm. At the beginning of today's inspection, there were 7 children (22 points) with the provider and two assistants. During the inspection, 3 more children arrived (8 points) for a total of 10 children (30 points) under the direct supervision of 3 caregivers. A sample of 3 children's files and 4 staff/household member's files were reviewed. Provider and assistants have exceeded the annual training hours required. The children were observed during the following: toy play, breakfast, hand washing, diapering, exercising, reading stories with the provider, lunch, bathroom breaks, hand washing, puzzles, transition to nap and nap time. The providers and the assistants were observed supervising the children, diapering, exercising with the children, redirecting children when needed and providing positive feedback. The physical space of the entire family day home and the playground area were inspected. Emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. There were no medications on site today. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-111-830-A
Description: Based on documentation review, emergency evacuation procedures were not practiced monthly.
Evidence:
1) On the date of inspection (12/02/2019), the emergency drills log had evacuation drills documented for the months of January 2019, February 2019, March 2019 and April 2019. No other drills were documented for the current year.
2) The emergency drills log for the previous year was not available for review.

Plan of Correction: I misplaced the current emergency drills log. Moving forward, I will make sure to keep it in a safe place and will start documenting the next drills.

Standard #: 22VAC40-111-830-B
Description: Based on documentation review, shelter-in-place procedures were not practiced a minimum of twice per year.
Evidence:
1) On the date of inspection (12/02/2019), the emergency drills log had a shelter-in-place drill documented for the month of January 2019. No other drills were documented for the current year.
2) The emergency drills log for the previous year was not available for review.

Plan of Correction: I misplaced the current emergency drills log. Moving forward, I will make sure to keep it in a safe place and will start documenting the next drills.

Description: Based on documentation review and interview, records of emergency evacuation and shelter-in-place drills were not maintained for one year.
Evidence:
1) On the date of inspection (12/02/2019), emergency evacuation and shelter-in-place drills records were not available for review.
2) Provider stated that she did not know that she had to keep the documentation for one year.

Plan of Correction: I was not aware of it. Moving forward, I will do it.

Standard #: 22VAC40-111-960-E
Description: Based on observation, bottles were not always labeled with and the date.
Evidence:
1) In the kitchen refrigerator, 7 out of 7 bottles were not labeled with the date.

Plan of Correction: I will make sure it gets done daily.

Standard #: 22VAC40-111-240-B
Description: Based on observation, recalled equipment was being used in the family day home.
Evidence:
1) An infant was observed being put in a Bumbo seat which was recalled in 2012.

Plan of Correction: It's a parent's seat. I will give it back today.

Standard #: 22VAC40-111-320-G
Description: Based on documentation review and interview, fireplaces and associated chimneys were not inspected annually by a knowledgeable inspector to verify that the devices are properly installed, maintained, and cleaned as needed.
Evidence:
1) On the date of inspection, (12/02/2019) the most recent fireplace inspection available for review was dated 08/23/2012.
2) Provider stated that the fireplace is not being used and she thought she did not have to have it inspected.

Plan of Correction: I will call to make the appointment. I will send the inspection report to the Licensing Inspector as soon as I have it.
