Today an unannounced Monitoring inspection took place between 9:50am and 11:30am. There were 5 children present with the provider and 2 assistant for a total of 13 points within the supervision guidelines. The children were observed practicing writing "ap" words in a word notebook, stacking cups, learning about colors and shapes from a learning DVD and playing freely. One child was observed napping during today's inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Standard #: 22VAC40-111-90-C Description: Based on record review, the family day home has not obtained additional immunizations every six months for children under the age of two.
Evidence: Child #5 (12 months old) most recent documented immunizations are from when the child was 2 months old.Plan of Correction: I will ask the parents today to bring in a copy.
Standard #: 22VAC40-111-180-A Description: Based on record review, the provider has not obtained for each caregiver a subsequent tuberculosis (TB) screening every two years.
Evidence: The providers most recent documented TB screening is dated 8/29/2017. Household member #3 most recent documented TB screening is dated 8/29/2017.Plan of Correction: We will get them done.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and interview, the provider has not obtained a central registry finding for each household member.
Evidence: Household member #3 file does not contain the results of a central registry background check. The family day home representative stated that she knows the background check was notarized and mailed, but doesn't know if results were received.Plan of Correction: We will check the email to make sure the results are there and will send them to inspector.
