Technical Assistance:

Reminded the provider that each caregiver must complete the Preservice Update training prior to 3/1/20.

Comments:

An unannounced full renewal inspection was conducted today. The provider and one assistant caregiver were caring for 4 children plus the provider's own child, totaling 14 points. Children's records, health records, caregiver training, adult records, background checks, children's nutrition, emergency preparedness items, child care programming, caregiver:child interactions, and the physical plant were inspected today. The children were observed actively engaged in a variety of activities including: creating art projects, playing with toys, playing with instruments, reading books, and preparing for lunch. The children were to be served red beans and rice, sliced apples, and milk for lunch today. This inspection was completed between approximately 10:00am and 12:00pm. Standard #:	22VAC40-111-790
Description:	Based on a review of posted administrative items, it was determined that the provider has not posted all required telephone numbers. Evidence: The provider has not posted the phone number for an emergency back up child care provider.
Plan of Correction:	I posted her address but forgot to include her phone number. I will add that information.
Standard #:	22VAC40-111-90-A
Description:	Based on a review of children's health records, it was determined that the provider did not obtain documentation that one child has been adequately immunized before the child began to attend care. Evidence: Child E entered care on 11/6/19. The provider has not obtained the child's shot record.
Plan of Correction:	I reminded the parent this morning. She is going to bring the shot record tomorrow.
Standard #:	22VAC40-111-240-A
Description:	Based on observations, it was determined that one outdoor area has not been maintained in a safe condition. Evidence: The Inspector observed the following: 1 - A full size ladder has been placed leaning against the wall next to the door where children leave the home to go outside to play. 2 - There are several cut pieces of wood and extra playground stakes on the ground next to the barrier that encloses the backyard play equipment. 3 - One of the portable slides/climbing cube has been placed directly next to the chain link fence and raise wood barrier.
Plan of Correction:	The ladder, wood pieces, and playground stakes will be moved before the children go outside today. I will also move the slide to a more open area on the playground.
