Comments:
An unannounced renewal inspection was conducted today from approximately 8:40am to 9:45am. 8 children (24pts) were present with 2 caregivers. Ratios were in compliance. The physical space, caregiver records, 3 children's records, attendance, the Emergency Preparedness Plan, 6 cribs, evacuation drills and required posted documents were reviewed. No medications were on site. Infant bottles were properly labeled and the program contained developmentally appropriate materials and activities. Children were observed singing, dancing and interacting with caregivers. Interactions were positive. Areas of non-compliance are identified in the Violation Notice. Description: Based on review of records and interview with Provider, 2 out of 3 records did not have a dated parent's signature to acknowledge an annual review of emergency contact information.

Evidence: The last review conducted, of emergency information, for Child #1 was on 8/30/2018 and for Child #2 was on 8/30/2018.

Plan of Correction: I will ask the parents to review the information and sign the child's record.
