Technical Assistance:
**Discussed one crib mattress that is currently not in use. If the crib is used in the future a mattress that fits snugly in the crib frame must be used.
The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated (December 2, 2019). Once the background check is received by the center, the results must be forwarded to the licensing inspector.

Comments:
An unannounced monitoring inspection was conducted today from approximately 11:40 am-1:30 pm. There were 9 children (22 points) directly supervised by 2 staff. The indoor child care area, 2 staff records, 6 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site.The provider is currently holding a valid medication administration certificate. Children were observed washing hands, eating a lunch consisting of grilled cheese sandwiches, mandarin oranges, green beans and milk. After lunch children were observed listening to a story and transitioning to nap . There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 4 of 6 children's records reviewed did not contain all of the required information.
Evidence:
1. Child 2's record did not contain the name of the child's health insurance carrier or policy number.
2. Child 4's record did not contain a health insurance policy number.
3. Child 5's record did not contain verification that the child's proof of birth was verified by the family day home.
4. Child 6's record did not contain the child health insurance policy number or verification that the child's proof of birth was verified by the family day home.

Plan of Correction: I will ask the parents to provide the missing information.

Standard #: 22VAC40-111-70-A
Description: Based on record review, one child's record did not contain documentation that the parents were provided in writing all required information.
Evidence:
Child 6's record did not contain the center's medication policy or
Provisions of the family day home's emergency preparedness and response plan.

Plan of Correction: I will collect the outstanding documents from parents.

Standard #: 22VAC40-111-180-A
Description: Based on record review, the provider did not obtain for each adult household member, a subsequent tuberculosis screening for caregivers and household members every two years from the date of the first screening or more frequently as recommended by a physician or the local health department.
Evidence:
The Tuberculosis screening for Adult 2 was completed on 9/7/2019.

Plan of Correction: The test has been completed and the form will be picked up on Thursday December 5, 2019.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review, the provider did not deny employment to caregivers who have not received a central registry finding within 30 days of employment.
Evidence:
Adult 3 (Hired 10/29/2019) does not have the results of a central registry check on file.

Plan of Correction: I have submitted the request and we are waiting for the central registry results.

Standard #: 63.2-1720.1-B-3
Description: Based upon documentation reviewed, an out of state child abuse and neglect result was not obtained.
Evidence:
One staff records reviewed states that Adult 3 has lived out of the state of Virginia in the preceding 5 years, and no out of state child abuse and neglect results were obtained (Staff (2); DOH 10/2019).

Plan of Correction: A request for the out of state background check will be completed and submitted.
