Comments:

An unannounced monitoring inspection was conducted December 4 2019 between 9:45am and 11:15am. There were 10 children in the direct care of the provider and two assistants. Children were observed washing hands, being fed, participating in circle time, singing, and napping. The family day home operates in 5 separate rooms to provide different spaces for the children to learn. The home is clean, organized, and offers age-appropriate materials for all ages in care. Standard #:	22VAC40-111-800-B
Description:	Based on observation, the provider did not ensure that the emergency plan was reviewed at least annually. Evidence: The provider stated that a review was conducted, but not documented to include the date of the review.
Plan of Correction:	I will document when staff discuss the procedures.
Standard #:	22VAC40-111-180-A
Description:	Based on record review, 1 of 3 staffing records did not contain documentation of a current Report of Tuberculosis Screening every two years from the date of the first screening. Evidence: Staff #1 record has documentation of a screening dated January 20, 2017.
Plan of Correction:	This will be completed next week.
