Violations:
Standard #: 22VAC40-111-750-D
Description: Based on observation, topical skin products were kept beyond their expiration date.
Evidence: Child #1 had two topical skin products observed at the family day home; both products expired 10/2019.

Plan of Correction: I threw the expired diaper cream out and I am sending the other cream home saying that I'll need a replacement.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence: Child #2 file did not contain the families medication insurance policy number.

Plan of Correction: I will text the family today to get the policy number.

Standard #: 22VAC40-111-180-A
Description: Based on record review and interview, the provider has not obtained for each household member a current Tuberculosis (TB) screening every two years.
Evidence: Household member #2 most recent documentation of a TB screening is dated 10/19/2016. The provider confirmed that a repeat screening has not been conducted.

Plan of Correction: I'll call the health department to get soonest possible walk-in appointment dates.
