54.1-3408 Provider must be MAT certified to administer prescription medication.

Comments:
The inspector conducted an unannounced mandated inspection from 2:00 until 3:30 p.m. There were eight children in care for a total of 18 pts. with the provider and an assistant present. Children were observed during rest time, and during diapering. Five children's records were reviewed and provider records were reviewed. All but one violation from the last inspection had been corrected. Please contact your inspector with any questions at 276-698-9981.

Description: Based on record review, the center did not obtain up to date documentation of immunizations.
Evidence:
1. The inspector reviewed five children's records. One of five (#2) records did not contain up to date immunizations, the lat immunization record was 10/12/18.

Plan of Correction: The provider texted the parent during the inspection asking for the the up to date records.

Standard #: 22VAC40-111-330-A
Description: Based on interview, the provider did not have a land line telephone.
Evidence:
1. The provider stated she does not have a land line.

Plan of Correction: The provider requested an allowable variance but it was denied; she just recently got the letter about it. provider will research options and get a land line telephone.
