Technical Assistance:
Discussion was held on the topics of current enrollment, and current status of household members.

Comments:
The following inspection took place between the hours of 9:30 am through 11:30 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Hendon, Virginia. At the start of this unannounced renewal inspection there were a total of 3 children observed (8 points) with 1 caregiver. A total of 3 children?s files, 1 staff file, and 7 household member?s files were reviewed. The children were engaged in the following: drop off, toy play, games, music, song, small motor skills, learning, and other organized activities. Violations:
Standard #: 22VAC40-111-780-B
Description: Based on review, 2 of 3 children files indicated that the Provider did not review annually with the parent the emergency contact information (as required in 22 VAC 40-111-60 B 2) to ensure the information was correct, and obtain the parent's signed acknowledgment of the review.

Evidence -
1. On the date of inspection (12/05/2019), the most recent documented review for Child #1 was dated - 11/20/2018. The most recent documented review for Child #2 was dated - 11/19/2018.
2. Both Child #1 and Child #2 were in attendance on the date of inspection.

Plan of Correction: This documentation shall be reviewed, signed, and dated by the parent of each applicable child. If needed, his information shall also be updated and placed on file.

Standard #: 22VAC40-111-830-C
Description: Based on review and interview, the documentation of shelter-in-place drills conducted within the past year were not recorded.

Evidence -
1. On the date of inspection (12/05/2019), the shelter-in-place drill log showed that the last practice drill was conducted on 08/14/2018.
2. The Provider stated (during an interview conducted on the date of inspection) that while two drills were conducted within the past year, neither drill was documented accordingly on this same log.

Plan of Correction: Will record.
