Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and expiration dates for background checks (the new expiration date is five years from the last finding date). Discussion was also made with regards to changing the age range of license to: 3 months through 4 years. The Provider was also encouraged to mail in her completed Family Day Home Renewal application, along with the renewal fee, that were viewed today on site.

Comments:
The following inspection took place between the hours of 8:30 am through 11 am. Today, I met with the Provider on site at the Family Day Home (FDH) located in Ashburn, Virginia. At the start of this unannounced renewal inspection there were a total of 9 children observed (28 points) with 2 caregivers. A total of 9 children?s files, 2 staff files, and 1 household member file was reviewed. The children were engaged in the following: toy play, snack, diapering, hand washing, song, music, dance, games, movie time, learning, and other organized activities. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review, 1 of 9 children files indicated that not all of the required documentation was complete and on file.

Evidence -
1. On the date of inspection (12/06/2019), a completed Medication Policy Form for Child #9 was not completed or signed by the parent of the child.
2. When reviewed, this form was blank on the signature page.

Plan of Correction: This form shall be completed and then signed by the parent of Child #9.

Standard #: 22VAC40-111-210-A
Description: Based on review, 2 of 2 staff files indicated that both caregivers did not obtain a minimum of sixteen (16) clock hours each of training annually in areas relevant to their job responsibilities.

Evidence -
1. On the date of inspection (12/06/2019), Staff #1 and Staff #2 each completed a total of 12 hours of annual training for 2019.
2. This total was confirm by the Provider on the date of inspection.

Plan of Correction: A total of 16 hours each shall be completed by the end of the 2020 annual training period.
