Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, December 09, 2019. The on-site inspection was held from 1:25 p.m. to 4:40 p.m. On this date, there were five children in the care of the provider (11 points). There were a total of eight children enrolled in the program. There were five children's records reviewed, along with the records for the provider and one household member. There were no medications being administered at the time of the visit, as is the policy of the provider. The exceptions to compliance were noted on the violation notice and the risk ratings for the violations were noted on the supplemental page. Afternoon snack was observed to meet USDA guidelines and consisted of crackers and bananas. Some items reviewed/observed during the inspection included: required posted information, emergency supplies, first aid kit, warm water temperature, refrigerator and freezer temperatures, documentation of evacuation and shelter-in-place drills, nap time, transitions, free time, diaper changing procedures, bathroom procedures, and snack.

Violations:
Standard #: 22VAC40-111-780-B
Description: Based on review of five children's records and verification from the provider, three children's records did not have written signatures from the parents that they had reviewed their emergency contact information to ensure the information is correct.

Evidence:
1. The record of Child #1 did not have documentation that the parent had reviewed enrollment information within a year of enrollment. Date of enrollment was 08/29/2018.
2. The record of Child #2 did not have documentation that the parent had reviewed enrollment information within a year of enrollment. Date of enrollment was 06/25/2018.
3. The record of Child #3 did not have documentation that the parent had reviewed enrollment information within a year of enrollment. Date of enrollment was 10/08/2018.

Plan of Correction: The provider will ensure that parents review enrollment information every year to ensure the information is correct.

Standard #: 22VAC40-111-180-A
Description: Based on review of the provider's record and the record of one household member and confirmation from the provider, both adults did not obtain an updated tuberculosis (TB) screening within two years from the most recent.

Evidence:
1. The provider obtained an updated TB screening on 10/15/2019. The date of the previous screening was 09/27/2017.
2. The household member obtained an updated TB screening on 10/23/2019. The date of the previous screening was 09/27/2017.
3. The provider confirmed they were late in getting their TB screenings.

Plan of Correction: In the future, the provider will ensure that they will obtain updated TB screenings within two years of the most recent TB.

Standard #: 22VAC40-111-680-B
Description: Based on observation and discussion with the provider, two children did not wash their hands before eating.

Evidence:
1. Two children sat down for afternoon snack and started eating without washing their hands.
2. The provider confirmed that she did not have the two children wash their hands before eating.

Plan of Correction: The provider will ensure that all children's hands are washed before eating.
