Comments:

An unannounced monitoring inspection was conducted today from approximately 12:45pm-2:20pm. There were 3 children (6 points) in the direct care and supervision of 1 caregiver. A sample of 2 children?s records, 1 caregiver's record, 8 household member's records, and 2 pet records were reviewed. The entire home was observed today. The children were observed napping and watching television. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. Description:	Based on record review, children's records were reviewed with the parent at least annually. Evidence: Child B's record was last reviewed 10/2018.
Plan of Correction:	I will ask her to review it today.
Standard #:	22VAC40-111-60-B
Description:	Based on record review, a child's record was missing required information. Evidence: Child A was missing the work phone number of the mother.
Plan of Correction:	She may have changed work, I will get it from her today.
