Technical Assistance:
Inspector spoke with provider about submitting a renewal application for the family day home during this inspection.

Comments:
An unannounced monitoring inspection was conducted today from 11:50 am to 12:30 pm. At the time of inspectors' arrival there was one child aged 18 months(3 points) in care with the provider. The child in care was observed interacting with the provider throughout this inspection. A sample of 1 child's records and 2 provider/household member records were reviewed. There were no injuries since the last inspection. Provider reported that no medications are administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the provider during the exit interview at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-90-C
Description: Based on record review it was determined the provider did not obtain documentation of additional immunizations every six months for children under the age of two years.
Evidence:
1. The record for child #1 did not contain documentation of additional immunizations every six months. The documentation available was date 5/31/19.
2. The provider confirmed that updated immunization documentation was not available.

Plan of Correction: Provider will request updated immunization documentation from the parent at pickup.

Standard #: 22VAC40-111-140-B-3
Description: Based on record review and interview it was determined the provider did not have current certification in cardiopulmonary resuscitation (CPR), as appropriate to the age of the children in care, from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current CPR certification issued within the past two years by a community college, a hospital, a rescue squad, or a fire department.
Evidence:
1. The record for the provider did not contain current CPR certification.
2. The provider stated they are scheduled to attend a CPR certification course on 12/11/19.

Plan of Correction: Provider will complete a CPR certification course.

Description: Based on record review and interview it was determined the provider did not have current certification in first aid from the American Red Cross, American Heart Association, American Safety and Health Institute, or the National Safety Council, or current first aid certification issued within the past three years by a community college, a hospital, a rescue squad, or a fire department.
Evidence:
1. The record for the provider did not contain current first aid certification.
2. The provider stated they are scheduled to attend a first aid certification course on 12/11/19.

Plan of Correction: Provider will complete a first aid certification course.

Standard #: 22VAC40-111-170-A
Description: Based on record review and interview it was determined the provider did not obtain from each adult household member a current Report of Tuberculosis Screening form published by the Virginia Department of Health or a form consistent with it documenting the absence of tuberculosis in a communicable form.
Evidence:
1. The record for the household member did not contain a Tuberculosis Screening form.
2. The provider confirmed that the household member has not received a Tuberculosis Screening.

Plan of Correction: Household member will get a tuberculosis screening or test.
