An unannounced complaint inspection was conducted today from 7:00 am until 9:00 am. The complaint investigation was in response to two allegations - 1. Morning ratios and 2. A new assistant without background checks. The inspector reviewed attendance logs and monitored attendance and ratios were with licensing standards. The inspector also reviewed all paper work for newly hired caregiver. Caregiver was hired 11/18/19 and has submitted Central Registry Background paperwork and is within the 30 day period for results to be returned. The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is deemed to be not valid. Description: Based on a review of children's records, the provider failed to annually review with the parent the emergency contact information required to ensure the information is correct and to obtain the parent's signed acknowledgement of the review.

Child #3's record did not have documentation of an annual review of the emergency information.

Plan of Correction: I will update information.

Description: Based on observation, an infant that could not hold their own bottle was not held for bottle feeding and had bottle propped.

An infant was observed in a swing with a bottle propped up on top of a small folded blanket for their feeding.

Plan of Correction: Corrected during inspection - I will not prop bottles.

Description: Based on observation, potentially poisonous substances such as cleaning agents, shall be stored away from areas accessible to children.

Bleach cleaning wipes were on the kitchen counter in an area accessible to children.

Plan of Correction: I will store cleaners out of areas accessible to children.

Description: Based on observation, the provider failed to ensure that small appliances shall be unplugged unless being used by the caregiver or with the children under close supervision.

A coffee pot was not unplugged and was not being used by the caregiver or with the children under close supervision.

Plan of Correction: I will remember to unplug small appliances when not in use.

Description: Based on a review of records, the provider failed to ensure that an annual inspection was conducted for wood burning fireplace.

The most recent wood burning fireplace inspection was conducted in September 2016.

Plan of Correction: I will schedule inspection as soon as possible.
