Technical Assistance:
Discussed hazardous substances, outside grill , rest areas and bottle labeling.

Comments:
Licensing inspector arrived at 11:25am and departed at 1:06 pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there was 8 children in care and ( 24 points) with 3 staff . Children were observed outside playing, eating lunch, watching tv, freely playing ad napping. The sample size consisted of seven children records and three staff/household records. Areas of non-compliance was reviewed with the provider.

The providers responses for the Description of Action To Be Taken were not received as of 12/12/2019 and will not appear on this Violation Notice.

Violations:
Standard #: 22VAC40-111-240-A
Description: Based on observation and staff interview, the provider did not ensure areas and furnishings of the family day home, inside and outside, were maintained in a clean, safe, and operable condition.

Evidence:
The door knob entering into the laundry room area was broke off. there was just a metal piece sticking out of the door.

Plan of Correction: The door knob will be replaced.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider did not ensure potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates were stored in areas inaccessible to children.

Evidence: In the bathroom, in the cabinet under the sink, there was Epsom Salt label said "Keep out of reach of children" and "Warning".

Standard #: 22VAC40-111-460-C
Description: Based on record review and staff interview, the provider did not ensure dogs or cats were vaccinated for rabies.

Evidence:
The most recent vaccination for the cat was 1/17/2017. The vaccination was good for only one year. The provider confirmed the vaccination was expired.

Plan of Correction: The provider will set up appointment by 12/12/2019.

Standard #: 22VAC40-111-520-C
Description: Based on observation and staff interview, the provider did not ensure have mattresses that fit snugly next to the crib so that no more than two fingers can be inserted between the mattress and the crib;

Evidence:
The cribs mattress were not fitted because of the bottom sheets would curl the mattress. Staff confirmed they were concerned about the mattress and sheets . Children were sleeping in the cribs when mattress were observed unfitted.

Plan of Correction: The provider will be purchasing new mattresses for all cribs.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider did not ensure the diapering surface was clean and sanitize after each use.

Evidence:
The provider did not clean the diapering surface with soap and water.
