Comments:
A mandated monitoring inspection was conducted today. There were 8 children present (21 points) with 3 caregivers. Children were observed having circle time, singing songs, learning their colors and shapes in Spanish and having child's choice. Reviewed 3 caregiver files, 2 resident files, 2 children's files, attendance, emergency evacuation log, crib compliance, medications and authorizations, menus, 1 pet file, and physical plant. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance.
Evidence:
Child#1 (start date 7/31/19) did not have documentation of a physical.

Plan of Correction: I will ask the parent for a copy

Standard #: 22VAC40-111-70-A
Description: Based on record review, before the child's first day of attendance, parents were not provided in writing the family day care home's policy regarding prescription and non-prescription medication administration.
Evidence:
Child#1 (start date 7/31/19) and Child#2 (start date 11/15/19) did not have documentation of receiving the medication administration policy.

Plan of Correction: I will give it to the parents and have them sign
