Comments:
A Subsidy Health and Safety inspection was conducted on December 12, 2019 to review supplemental health and safety requirements. Provider was present with four children for a point total of eleven. Four children records and provider record were reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-400-A
Description: Based on interview, provider did not ensure there is a written emergency preparedness plan that addresses all required components.

Evidence: Provider could not provide the written procedures regarding accommodations for infants/toddlers and children with special needs to ensure their safety, relocation, lockdown in the emergency preparedness plan. Provider stated she printed off the state model form but she has not filled it out.

Plan of Correction: Provider will complete the forms with all required information.
