Comments:
A Subsidy Health and Safety inspection was conducted on 12/12/19 to review supplemental health and safety requirements. There were five children in care with a total of 12 points. The provider and assistant provider were present. Five children's records were reviewed during this inspection, including one new children's records. All provider records were reviewed, including one new assistant provider record. This family day home does not administer medications or provide transportation. The inspector arrived at the family day home at 9:00 AM and departed at 12:00 PM. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and discussed at the conclusion of the inspection. Standard #: 22VAC40-665-190-A
Description: Based on record review, the provider failed to ensure that staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.

Evidence:

1. Staff 2 had a documented date of hire, 11/22/2019 and a dated documentation of TB screening, 12/10/2019. Staff 1 verified that Staff 2 started in November of 2019.

Plan of Correction: The provider will make herself aware of this for her next hire.
