Comments:
An unannounced mandated inspection was conducted on 12/12/2019. The inspection began at 9:00 AM and concluded at 12:00 PM. There were five children in care with a total of 12 points. The provider and her assistant were present on this day. During the inspection, children were observed during free play, letters and numbers comprehension, and coloring. The inspector reviewed five children's records, including one new record. The inspector reviewed one new assistant record and all other household members' records and all caregiver records had no changes. The inspector examined rooms used for care, drill documentation, and the emergency supplies. This provider does not administer medications. There was discussion about medication administration. Standard #: 22VAC40-111-730-D
Description: Based on observation and discussion with the provider, the provider failed to ensure medication, except for those prescriptions designated otherwise by a written physician's order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:

1. A bottle of an antihistamine medication was unlocked and located on a top shelf in the front room; not accessible to children.

Plan of Correction: The provider will make sure that all medications are locked.

Standard #: 22VAC40-111-750-A
Description: REPEAT

Based on observation and discussion with the provider, the provider failed to ensure that when topical products such as sunscreen, diaper ointment and lotion, oral teething medicine, and insect repellent are used, all requirements of the standards shall be met.

Evidence:

1. Located on an upper shelf, containers of "healing ointment" and a spray bottle of sunscreen were identified by the provider as a child's but was unlabeled.

Plan of Correction: The provider will label the items.

Standard #: 22VAC40-111-60-B
Description: Based on record review, the provider failed to ensure that a child's record shall contain all the elements as required by the standards.

Evidence:

1. The record for Child 1 was missing the following:
A. School or concurrent program
B. The address for the child's mother and father did not contain the complete street address.
C. Emergency contact number 2 did not contain a complete street address.
D. Written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately was not signed or dated by the parent(s).

Plan of Correction: The provider will obtain this information from the parents.

Standard #: 22VAC40-111-280
Description: Based on observation, the provider failed to ensure that potentially poisonous substances, materials and supplies such as, but not limited to, cleaning agents, disinfectants, deodorizers, plant care chemicals, pesticides, and petroleum distillates shall be stored away from food in areas inaccessible to children.

Evidence:

1. A cleaning agent spray was located in the floor-level bathroom cabinet, which was unlocked and accessible to children.

Plan of Correction: Provider removed item during the inspection.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to ensure that the diapering surface shall be nonabsorbent.

Evidence:

The diapering pad had a small tear in it, exposing the absorbent material inside the pad. A child was observed being changed on the pad during the inspection.

Plan of Correction: Provider will purchase a new diaper pad.

Standard #: 22VAC40-191-60-C-2
Description: Based on record review and discussion with the provider, the provider failed to obtain the central registry findings within 30 days of employment or volunteer service.

Evidence:

1. The record for Staff 3 did not contain central registry findings for the state of Virginia. The provider verified that Staff 3 has been working for her for more than a year.

Plan of Correction: The provider will send off for Central Registry paperwork.
