Comments:
An unannounced monitoring inspection was conducted today from 1:50pm to 3:30pm with the provider. There were four children in care, ranging from two to four years of age, for a total of eight points. One assistant was on break. Children were observed napping. The playground was not observed due to rain. Four children?s records, the provider's record, one assistant's record, three household members? records, and one pet record were reviewed. The provider and assistant have current certification in CPR and First Aid. The family day home does not have any medication at this time and the provider is aware that she cannot administer any medication until she renews her Medication Administration Training on 12/14/19. The emergency drill log, attendance, and menu were also reviewed. Snack today will be honeydew melon and crackers. The last documented evacuation drill was dated 11/25/19 and the last documented shelter-in-place was dated 6/28/19. If you have questions regarding this inspection, please contact the Licensing Inspector, Laura Brindle, at laura.brindle@dss.virginia.gov or 540-905-2062.

Violations:
Standard #: 22VAC40-111-50-A
Description: Based on review of documentation and interview with the provider, the attendance did not include all children who were in care. Evidence: On 12/10/19 at 1:50pm there were four children in care. The written attendance record only had documentation of three children in care. The provider stated that one child arrived around 11:30 and had not been added to the record.

Plan of Correction: I added the child during the inspection and in the future I will make sure I update the attendance as children arrive.

Standard #: 22VAC40-111-180-A
Description: Based on review of three household member records, the provider did not obtain documentation of the results of a Tuberculosis (TB) test or screening every two years from the date of the first screening for each household member. Evidence: The most recent documentation of a TB test or screening for Household Member A was dated 6/21/17. The most recent documentation of a TB test or screening for Household Member B was dated 8/17/17.

Plan of Correction: I have already asked them to go get another test.
