Comments:
An unannounced renewal inspection was conducted today between 9:20 am and 11:30 am. There were 7 (16 points) children present with 3 caregivers. The children were observed eating breakfast, working on puzzles and exercising. Hand washing and diaper changing was also observed. The physical plant, attendance records, emergency plants, emergency supplies, evacuation records, cribs, children's records, caregiver records, household members records and menu's were inspected. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records, the provider failed to obtain documentation of physical examination by or under the direction of a physician prior to child's attendance or within 30 days after the first day of attendance.

Evidence:
Child #4's records do not contain documentation of a physical by the 30th day of attendance.

Plan of Correction: Provider contacted the parent during the inspection, parent will drop off form tomorrow.

Standard #: 22VAC40-111-60-A
Description: Based on a review of children's records, the provider failed to ensure that records are up to date for each child enrolled in the family day home.

Evidence:
Child # 2 and Child #4's records do not contain the name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached;

Plan of Correction: I will ask the parents for the information and put in the child's file.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on a review of household members records, the provider failed to obtain a Child protective services central registry check within 30 days of a person in the home becoming 14 years old.

Evidence:
Household member #3's record did not contain documentation of a Child protective services central registry check.

Plan of Correction: I will obtain and submit CPS check for a household member that has turned 14 years old.
