Technical Assistance:
** Discussed upcoming Tuberculosis screening expiration dates.
***The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated 12/17/2019. Once the background check is received by the center, the results must be forwarded to the licensing inspector.
****Discussed background check requirements for household members after the age of fourteen.

Comments:
An unannounced monitoring inspection was conducted today from 11:45am and 1:00pm. There were 7 children (18 points) directly supervised by the provider and assistant. The child care area, 2 staff records, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed playing blocks with caregivers and watching a children's television program. Staff training was inspected and staff have completed thirty hours of annual training which exceeds the requirement by sixteen hours.. Diapering procedures were also observed. There was an assortment of age appropriate materials for children. The home was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 5 of 5 children's records reviewed did not contain all required documentation.
Evidence:
1. The records for children 3 and 4 did not contain the name, address, and phone number of a second person to contact in the event that a parent cannot be reached.
2. The records for children 1, 2, 4, and 5 did not contain documentation of an annual review.

Plan of Correction: I will ask the parents to update the records.

Standard #: 22VAC40-111-530-B
Description: Based on observation, two of the three cribs observed did not have a tight-fitting bottom cover.
Evidence:
Two cribs were observed with cribs sheets that did not fit tightly.

Plan of Correction: I will tighten the crib sheets.

Standard #: 22VAC40-191-40-D-1-c
Description: Based on record review, one adult caregiver did not have a sworn statement or affirmation and search of central registry conducted before five years since the dates of the last sworn statement or affirmation and most recent central registry finding.
Evidence:
1. Adult 1's most recent sworn statement was completed on 10/14/2014.
2. Adult 1's most recent central registry check was completed on 10/30/2014.

Plan of Correction: I will complete new background checks.
