Comments:
An unannounced renewal inspection was conducted today from approximately 11:40 am to 12:55 pm. There were 4 children (8 points) in care of the provider.

4 children?s, 3 staff records, 1 household member record, caregiver training, children?s nutrition, supplies, emergency procedures and drills were inspected. The children were observed playing freely and transitioning to nap time. There were age appropriate materials for all children in care. Hand washing was observed. Areas of non-compliance are identified in the Violation Notice.

Standard #: 22VAC40-111-100-A
Description: Based upon review of 1 out 4 children records, the provider did not obtained documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #4 (D.O.E. 8/14/2017), physical examination results were not available in the child?s record.
2. The provider verified that she had not received physical examination from the parent for Child #4.

Plan of Correction: Contacted parent to finish physical examination of child in subject
