Comments:
A mandated monitoring inspection was conducted today. There were 10 children present (29 present) with 2 caregivers. Children were observed having lunch (potato latkes, grapes, carrots and milk) having diapers changed, hand-washing, bottles being fed, a book read and nap time. Reviewed 4 children's files, 2 caregiver files and 1 resident file, attendance, menus, emergency drill log, crib compliance, and physical plant. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by a physician within 30 days after the first day of attendance.
Evidence:
Child#1 (start date 8/13/19) did not have documentation of a physician.

Plan of Correction: I will ask the parent for a copy

Standard #: 22VAC40-111-70-A
Description: Based on record review, parents were not provided whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home by the child's first day of care.
Evidence:
Child#3 (start date 5/20/19) did not have signed acknowledgement of receiving the family day care homes liability insurance status.

Plan of Correction: I will have the parent sign
