Comments:
An unannounced renewal inspection was completed on 12/18/19. There were 12 children present with the provider and two assistant caregivers supervising. The point total was 28. Two newly enrolled children's records were reviewed and seven other children's records were reviewed for annual updates only. All caregiver and household records were up-to-date; caregivers records were reviewed for annual training only. No medications are administered to the children in care.

The inspector arrived at the provider's home at 8:30 am and departed at 1:00 pm.

The children were observed during arrivals, during free play times with various activities, during structured learning activities, during story time, and during lunch.

Violations:
Standard #: 22VAC40-111-530-F
Description: Based on observation, the provider failed to ensure that no soft bedding of any kind shall be used under or around infants including, but not limited to, pillows, quilts, comforters, sheepskins, or stuffed toys.

Evidence:
1. At arrival of the inspector, a three month old infant was observed sleeping on top of a large stuffed pillow that was placed in the crib. The pillow was approximately three inches thick and round in shape taking up approximately 1/2 of the crib area lengthwise and approximately 3/4 of the width of the crib. The infant was observed sleeping on the pillow for approximately 10 minutes when the inspector notified the provider of the Standard requirements prohibiting the use of pillows for infants.

2. The inspector observed the manufacturer's recommendations on the attached tag on the pillow which stated "Do not allow baby to sleep in this product." This statement had a symbol There were also other precautions stated on this tag for use. .

Plan of Correction: The provider removed the infant from the pillow when the inspector notified the provider of the requirements of the Standards.

Standard #: 22VAC40-111-570-A
Description: Based on observation, record review, and interview, the provider failed to ensure that a caregiver did not exceed 16 points using the point system provided by the Standards.

Evidence:
1. At arrival of the inspector, the provider was the only caregiver present with seven children in care. The ages of the children were three months through four years old and the point total was 18. Two additional children (two years and four years) arrived at approximately 8:35 am which increased the point total to 22 with the provider as the only caregiver present.

2. The provider stated that two children (four points) arrived earlier on the date of the inspection than their scheduled arrival time which is normally 9:30 am or later.

Plan of Correction: This was corrected when an assistant caregiver arrived at approximately 8:50 am. A second assistant caregiver arrived at approximately 9:00 am.

Standard #: 22VAC40-111-600-B
Description: Based on observation, the provider failed to ensure that infants and toddlers did not spend more than 30 minutes of consecutive time during waking hours, with the exception of mealtimes, confined in a crib, play pen, high chair or other confining piece of equipment. The intervening time period between confinements shall be at least one hour.

Evidence:
1. A three month old infant was observed sleeping in a crib at arrival of the inspector at 8:30 am. The infant woke up at approximately 8:45 am and stayed in the crib until approximately 8:57 am. The infant was placed in a floor swing with a safety strap which is a confining piece of equipment at approximately 8:57 am. The infant was observed awake and still in the swing at 9:30 am.

2. The same three month old infant was observed being placed in a bouncy seat with a safety strap which is a confining piece of equipment at approximately 11:30 am. The infant was awake from the time being placed in the bouncy seat and was still awake and in the bouncy seat at 12:15 pm. The infant was in the confining equipment for more than 30 minutes while awake.

Plan of Correction: The infant was taken out of the swing at approximately 9:45 am and allowed one hour of time outside of a confining space at that time. The infant was removed at approximately 12:30 pm. The provider will make sure that the infant is taken out of any confining equipment after 30 minutes while awake and allow at least one hour of non-confining time in between.
