Technical Assistance:
During the inspection, the licensing representative discussed with the proivder:
-current enrollment and substitute provider
-fire safety and shock prevention
-diapering changing

Comments:
An unannounced monitoring inspection was conducted today approximately 10:240 am- 3:15 pm. There were 13 children (39 points), 2 of the children were the provider?s children. The children were directly supervised by 4 staff. The physical plant, 5 staff records, 3 household records, 7 children?s records, children's nutrition, supplies, evacuation drills, injury reports, emergency supplies, and policies were inspected. The children were observed in free play and in center activities. Diapering changing and hand washing was observed during the inspection. There were age appropriate materials for all children. Staff-to-child ratios were in compliance.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on children's record review, 5 out 11 records did not contain the required information in the records.
Evidence:
1. Child #1 records did not have documented second emergency contact to include name, address, and phone number and parent signature information for parents.
2. Child #2 record did not have documented second emergency contact to include name, address, and phone number, authorized pick-up person, immunization of the first day of attendance and a physical within 30 days of attendance.
3. Child #3 record did not have documented second emergency contact to include name, address, and phone number and authorized pick-up person.
4. Child#4 record did not have annual review update.
5. Child #7 did not have a record.
6. Child #11 did not have a record, but Child #11 have a sibling with a complete record on file.

Plan of Correction: Will have parent complete missing information.

Standard #: 22VAC40-111-680-B
Description: Based upon observations, the provider did not ensure hands were washed with liquid soap and warm running water after toileting or diapering.
Evidence: Staff #5 changed one child's diaper. The child 's hands were not washed after diapering.

Plan of Correction: Make sure they do the requirements.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider did not ensure the diapering surface was nonabsorbent, cleaned and sanitized.
Evidence: Staff #5 changed a child diapers. Staff #5 did not clean the surface and sanitized the surface using Lysol disinfecting spray. The diapering surface was not cleaned and sanitized after each use.

Plan of Correction: I will have staff meeting today to re-explain rules of sanitation.

Standard #: 22VAC40-191-60-C-2
Description: Based on staff records review, 1 out 5 records, the provider did not obtain central registry fining within 30 days of employment.
Evidence: 1. The provider stated Staff #3 was hired January 1, 2019. A central registry results was not on file. A central registry result has to be completed within 30 days of hire.
2. The provided stated contacted OBI to request information and have not received information. The central registry check was resubmitted on November 14, 2109.

Plan of Correction: I resubmitted on November 15, 2019, currently waiting on paperwork.
