Comments:

An unannounced monitoring inspection was conducted with the provider and assistant beginning at 8:35 AM and ending at 9:40 AM. There were three children in care during the inspection. The children were ages 8 months, 3, and 4 years old for a total of 8 points. The infant was observed in a bouncy seat. The older children were observed watching an age appropriate television show and playing with toys. Four children?s records and three staff records were reviewed. There were no medications on site as the provider is not certified in Medication Administrative Training (MAT). The emergency preparedness plan and emergency drill logs were reviewed. The most recent emergency escape drill was documented as 11/24/19. The most recent shelter in-place drill was documented as 10/10/19. Description:	Based on a review of documentation, not all of the children's records contained all the required informations. Evidence: Child A's record was missing the name, address, and phone number for the parents' place of employment.
Plan of Correction:	The provider has notified the parent that they needed the missing information. The provider copied the form for the parent to take home and fill in the missing information.
Standard #:	22VAC40-111-90-A
Description:	Based on a review of documentation, the provider did not have documentation of a immunization for all children prior to first day of attendance. Evidence: Child A, start date 12/2/19, did not have documentation of immunizations in their records.
Plan of Correction:	The provider stated that she has already spoken to the parents regarding the missing documentation and they are working on obtaining the documentation of immunizations.
