Comments:
The licensing inspector conducted an unannounced renewal inspection on Wednesday, December 11, 2019 from 11:25 AM until approximately 2:30 PM to determine the provider?s compliance with licensing standards. There were 10 non-resident children in attendance being cared for by two caregivers. Based on the children?s ages there was a total of 25 points. The children were observed playing with toys, watching children?s programs on T.V., singing songs, infants being held, rocked and fed, taking bathroom breaks and washing hands. Six children?s records, two caregivers? records, and one household member?s records were reviewed during the inspection. There is one household pet and necessary documentation was reviewed and found to be in compliance. The most recent evacuation drill was documented on 11/04/2019. The two most recent shelter-in-place drills were documented on 7/8/2019 and 12/3/2019. A lockdown drill was documented on 12/9/2019.

Violations:
Standard #: 22VAC40-111-800-B
Description: Based on the inspection of the emergency preparedness plan, the provider did not ensure to review the emergency plan at least annually and update the plan as needed. The provider shall document in writing each review and update to the emergency plan.

Evidence: 1) During the inspection of the emergency preparedness plan, it was discovered the plan was not reviewed and updated annually with documentation maintained for review.

Plan of Correction: The provider reported the emergency preparedness plan would be reviewed on 12/11/19 and documented as required. In the future, the emergency preparedness plan will be reviewed annually.

Standard #: 22VAC40-111-90-A
Description: Based on a review of six children's records on 12/11/2019, the provider did not obtain documentation that two children had been adequately immunized before the children attended the family day home.

Evidence: 1) The record for Child #4, whose date of attendance was 09/20/2019, did not contain documentation of the child's immunizations. 2) The record for Child #5, whose date of attendance was 09/20/2019, did not contain documentation of the child's immunizations. Before a child may attend the family day home, the provider shall obtain documentation that the child has been adequately immunized according to the requirements of ? 32.1-46 A of the Code of Virginia and applicable State Board of Health regulations.

Plan of Correction: The parent was contacted while the inspector was on site and a request made for the immunizations to be provided by the following day.

Standard #: 22VAC40-111-330-A
Description: Based on observation and interview on 12/11/2019, the provider did not ensure to have a land line telephone, excluding a cordless or cell phone, available, operable, and accessible during the family day home's hours of operation.

Evidence: 1) During the inspection, the licensing inspector asked to see the provider's landline telephone. The provider stated there was no working land line telephone available in the home at the time of the inspection.

Plan of Correction: A new land line telephone will be purchased today. In the future, the provider will ensure there is a working land line telephone on site during the family day home's hours of operation.
