Comments:

An unannounced renewal inspection was conducted today between the hours of 2 pm through 4:30 pm with the provider on site. There were 7 children (16 points) with the provider and 2 caregivers. A sample of 5 children's files and 5 caregiver/household member's files were reviewed. The children were observed during snack time, story time, making Christmas cards, and playing with toys. The physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. Description:	Based on record review, the provider did not review with the parent the emergency contact information required in 22 VAC 40-111-60 B 2 to ensure the information is correct and obtain the parent's signed acknowledgment of the review for 2 of 5 children. Evidence: 1. Child #1 began attending the family day home on 12/4/17. The last signed review of emergency contact information documented in the record was dated 11/7/18. 2. Child #2 began attending the family day home in August 2018. There was no documented review of emergency contact information available for review.

Description:	Based on record review, 2 of 3 caregiver records did not contain all required information. Evidence: 1. The record for Caregiver #1 did not contain date of employment and verification of age. 2. The record for Caregiver #2 did not contain date of employment.

Description:	Based on record review and caregiver interview, 2 of 5 children's records did not contain all required information. Evidence: The record for Child #1 did not contain names of persons other than the custodial parents who are authorized to pick up the child. 2. The record for Child #2 was missing the following information: - first date of attendance - proof of the child's age and identity -written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately Caregiver stated the document which contains the missing information for Child #2 was given to the child's emergency contact and had not been returned to the family day home at the time of inspection.
