Comments:
An unannounced renewal inspection was conducted today between the hours of 9:30am through 11:30am with the provider on site. There were 5 children (11 points) with the provider and assistant. A sample of 5 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, coloring, sorting noodles into bowls, snack, hand washing, and other organized activities. . Interactions between the children and caregivers were positive. The physical space, emergency supplies, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. Standard #: 22VAC40-111-880
Description: ased on review of the menu, the family day home is not following the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
Evidence:
1. The provider emailed the parent's of the children a menu for the week that did not include on the required components. Friday's lunch is listed as five cheese ravioli and apple juice.

Plan of Correction: I will add milk and broccoli to the menu on Friday.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.
Evidence:
1. Child #3 began with the provider on 6/24/2019 and did not have documentation of a physical examination within 30 days after the first day of attendance.

Plan of Correction: I will ask the parent today.

Standard #: 22VAC40-111-70-A
Description: Based on review of five children's records, the provider did not provider in writing the provisions of the family day home's emergency preparedness and response to all children in enrolled before the child's first day of attendance.
Evidence:
1. Child #2 began with the provider on 1/02/2018 and did not have the Provisions of the emergency plan in the child's file and did not have the parent's written acknowledgment.

Plan of Correction: I will have the parent review and sign today..

Standard #: 22VAC40-111-320-B
Description: Based on observation, child-resistant protective covers were not installed on all unused electrical outlets and surge protectors accessible to children under five years of age.
Evidence:
1. One unused outlet under the window was not covered in the main play area was not covered with a protective cover.
2. One unused outlet under the welcome sign was not covered in the main play area was not covered with a protective cover.
3. One unused outlet to the left of the closet in the baby area was not covered with a protective cover..

Plan of Correction: The outlets were covered during the inspection.
