Technical Assistance:
The next anticipated inspection will be the provider?s renewal inspection. Provider was reminded to have all renewal application documents completed and sent into licensing office 45 days prior to expiration for processing. The renewal application is available on the VDSS website.

Comments:
Today an unannounced Monitoring inspection took place between 11:50am and 1:15pm. There was 1 child present with the provide for a total of 4 points within the supervision guidelines. The children was observed playing independently, having their diaper changed, receiving a bottle and being laid down to nap. A complete inspection of the physical plant, children and staff records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Violations:
Standard #: 22VAC40-111-830-A
Description: Based on record review and interview, emergency evacuation procedures were not practiced monthly.
Evidence: There is no drill documented for the month of November. The provider confirmed that she must have forgotten to do one.

Plan of Correction: I'll remember to do it.

Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider has not obtained a physical exam within the first 30 days of attendance for one child.
Evidence: Child #1 (date of enrollment: 9/3/2019) file does not contain documentation of a physical exam.

Plan of Correction: I'll request this form from the child's family.

Standard #: 22VAC40-111-60-B
Description: Based on record review, children's records did not contain all required information.
Evidence: Child #1 file does not contain the child's medical insurance company or policy number. Child #2 file does not contain the child's medical insurance policy number.

Plan of Correction: Child #1's insurance information was obtained during the inspection. I will get the missing insurance number from Child #2's family.

Standard #: 22VAC40-111-90-A
Description: Based on record review, the provider has not obtained additional documentation of immunizations once every six months for children under the age of two years.
Evidence: Child #2 (date of birth: 4/6/2018) most recent immunizations are dated 1/7/2019.

Plan of Correction: The parent contacted the pediatricians office during today's inspection to request this information.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider did not ensure that children's hands were washed after diapering.
Evidence: Child #1 was observed having their diaper changed. The provider did not wash the child's hands after the diaper change was complete.

Plan of Correction: I am always aware of where their hands are and if they come anywhere near anything that has come anywhere near the diaper area I wash them. In this case that didn't happen. I will always wash them in the future.
