Technical Assistance:
The following FDH standards were reviewed:
*Standard 500.D.4 - Openings above the ground that are closed on all sides shall be smaller than 3-1/2 inches or larger than nine inches to prevent head entrapment hazards;
*Standard 610.A - Use of media such as, but not limited to, television, videos, video games, and computers shall be limited to not more than a total of two hours per day;
*Standard 500.F - Trampolines shall not be used during the hours children are in care;
*Standard(s) 980 - 1010 - Transportation and Field trips;
*Standard 210 - Caregivers shall obtain 16 clock hours of training annually;
*Standard 220 - Prescription and non-prescription medication may only be administered by a caregiver with current medication administration training (MAT).
The inspector additionally reviewed the following:
Injury documentation and notification, child abuse reporting ( required within 24 hours of suspicion), USDA requirements, children and caregiver records, caregiver qualifications, linens, rest mats and the inspection process to include the risk matrix and the terms of the license.
*The current license is requested to be posted in an area accessible to the public along with the most recent licensing inspection reports. ( inspection summary & violation notice).

Comments:
An announced initial inspection was conducted on 12/30/2019 from 11:50 am - 2:31 pm. At the time of entrance there were two children in care with two caregivers. The ages of the children counted for a total of four points. The inspector observed the children watching cartoons on the television in the child care room. One diaper change was observed. The lunch served consisted of beef & vegetable pot pie, peaches, and juice.
The backyard play area was reviewed in addition to the trailer and breeze way area utilized for child care.
Violations were observed in six parts of the FDH standards and are listed on the violation notice. These violations were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-800-A
Description: Based on interview, the provider failed to have a written emergency preparedness and response plan for the family day home.

Evidence:
The provider stated that she did not have a written emergency preparedness and response plan for the family day home.

Plan of Correction: The emergency plan will be written.

Standard #: 22VAC40-111-880
Description: Based on observation, the provider failed to ensure that when meals are provided the family day home shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).

Evidence:
The lunch served to children did not include fluid milk as required by USDA for all meals.

Plan of Correction: Milk will be served at each lunch going forward.

Standard #: 22VAC40-111-920
Description: Based on observation, the provider failed to ensure that when meals or snacks are provided by the family day home, the menu for the current one-week period shall be posted in an area accessible to parents.

Evidence:
1. The current menu was not posted.
a. The provider verified that she provides meals and snacks for the children each day.

Plan of Correction: The menu will be posted.

Standard #: 22VAC40-111-120-B
Description: Based on record review, the provider failed to ensure that caregiver records contain all of the required elements.

Evidence:
1. The record for caregiver 2 lacked the following required information:
a. Verification of age;
b. Date of employment or volunteering;
c. Name, address and telephone number of a person to be notified in an emergency;
d. Documentation of a second reference check as to character and reputation as well as competency.

Plan of Correction: The record for caregiver 2 will be put together by the close of business tomorrow.

Standard #: 22VAC40-111-200-C
Description: Based on record review and interview, the provider failed to maintained documentation of orientation training which shall be signed and dated by the provider and assistant.

Evidence:
1. The provider stated that she did not have written documentation to demonstrate that orientation training had been provided to caregiver 2.

Plan of Correction: Orientation training will be documented for caregiver 2.

Standard #: 22VAC40-111-240-A
Description: Based on observation, the provider failed to ensure that outside areas of the family day home shall be maintained in a safe condition.


Evidence:
Two nails were observed protruding from two loose areas of lattice on the bottom area of stairway to the outdoor entry/exit door to the backyard play area.

Plan of Correction: The lattice will be secured to the railing.

Standard #: 22VAC40-111-290
Description: Based on observation, the provider failed to ensure that sharp kitchen utensils shall be inaccessible to children.

Evidence.
1. The child proof latch on the kitchen drawer, beside the stove, is loose allowing the drawer to easily open.
a. A bread knife was observed inside of this drawer which is accessible to the children in care.

Plan of Correction: The child proof latch will be secured.

Standard #: 22VAC40-111-500-C
Description: Based on observation, the provider failed to ensure that stationary outdoor playground equipment shall be placed at least six feet from the obstacles.

Evidence:
A plastic little tikes play structure with slide was placed directly beside an area of trees and shrubbery.

Plan of Correction: This play structure will be moved today.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the provider failed to ensure that children's hands are washed with liquid soap and warm running water before eating.

Evidence:
The provider did not wash the hands of the two children in care prior to lunch at approximately 1:30pm.

Plan of Correction: Children's hands will be washed with soap and water prior to eating lunch.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the provider failed to ensure that the diapering surface shall be nonabsorbent and washable and cleaned after each use.

Evidence:
1. The provider changed the diaper of child 1 on a floor mat that was placed on the floor. This mat is on an area of floor that is walked on when accessing the desk area of this room.
a. After the diaper change the provider did not clean the floor mat with soap and water.

Plan of Correction: A diapering mat or pad will be purchased and the diapering surface will be cleaned with soap and water after each diaper change.
