Technical Assistance:
Licensing inspector discussed requirements for closing the facility with provider after initial attempt on 12/23/2019.

Comments:
A Subsidy Health and Safety inspection was conducted on 12/30/2019 to review supplemental health and safety requirements.
An unannounced monitoring inspection was conducted today from approximately 2:50pm-3:45pm. There were no children in care. A sample of 2 children?s and 2 staff records were reviewed. The facility is getting the kitchen remodeled and is closed. Areas of non-compliance are identified in this report. The exit interview was conducted with the provider. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Description: Based on record review, the lockdown procedures were not practiced at least annually.

Evidence:
The lockdown procedure was not completed in 2018.

Plan of Correction: I will complete it by the end of this year.
