Comments:
An unannounced monitoring inspection was conducted today, December 16, 2019 to determine the providers compliance with family day home standards. The inspection was initiated at 9:15 am and concluded at 11:30 am. Upon the inspector's arrival there were seven children in care with one provider for a value of 15 points. Children were playing with the kitchen set, sensory table and puzzles. Later the assistant/household member stepped in and reviewed flash cards with the children and played a group game with the parachute. The family day home is equipped with age appropriate materials and supplies. Caregivers interacted with the children in a positive manner. The childcare area is decorated with seasonal decorations. All areas used for child care were inspected along with a full walk through of the home. The pool ladder was locked and prevented access to the pool. The first aid kit and non-medical supplies for the home was inspected and found complete. The family day home had the required information posted to include; license, most recent inspection notice, emergency numbers, daily schedule, allergy list, menu and calendar of activities. Today's lunch listed was chicken patties, WG bread, green beans, mixed fruit and milk. The inspector interacted with the provider and children in care.

Two caregiver records, one household resident's record and seven children's records were reviewed.
The most recent evacuation drill was documented on 11/19/2019. The two required shelter-in-place drills for 2019 were completed on 03/18/2019 and 09/05/2019. A lock down drill was completed on 03/19/2019.
Per the Provider: Currently no medications are being administered.
Caregivers have current CPR and First aid. One caregiver has MAT certification.
Violations cited during the licensing period were checked for corrections. There were no repeat violations cited during today's inspection.

Standard #: 22VAC40-111-70-B
Description: Based on review of children's records, the provider did not obtain the parent's written acknowledgement of the receipt of information required to be provided by the child's first day of attendance.

Evidence:

1. The record of child #3, enrolled 10/21/2019, did not contain documentation of the parents written acknowledgement of receiving the required "Information for Parents".
2. The provider indicated that the child's parent had been provided the information prior to the child's first day, but she had not yet obtained the signed acknowledgement.

Plan of Correction: Per the provider: I will have the parent sign the acknowledgment today at pick up.
