Comments:
An unannounced monitoring inspection was conducted with the provider. There were four daycare children in care along with the provider's child. The total number of points were 14. Children were observed during lunch time, free play and rest time. Three children records and one house hold member's records were reviewed. The inspection time was 10:30 am to 12:45 pm. Standard #: 22VAC40-111-50-A
Description: Based on review there was no written record of the children in care.
Evidence: There was no written record of the children that were in attendance:

Plan of Correction: As soon as the parents gets back from holidays which is tomorrow.

Standard #: 22VAC40-111-180-A
Description: Based on review of the provider's and one household members record did not have a T.B. test before two years since the last test.
Evidence: The date on the most current T.B. test was 11/24/2017. For the provider and House hold member A.

Plan of Correction: House hold member A did the T.B. test today. I the provider will do the test this week.

Standard #: 22VAC40-111-590-A
Description: Based on observation and interview with the provider, not all infants were placed in cribs on their back to sleep.
Evidence: One three month old infant that cannot roll over was placed on the stomach to sleep.

Plan of Correction: The infant was placed on her back as soon as her next nap. It was fixed right away.
