Comments:

A unannounced renewal inspection was conducted with the provider and one assistant. One child was in care during the inspection that was two years old. The child was observed during rest time, free play and snack time. Seven children's records were reviewed. One assistant and five house hold member's records were also reviewed. Please remind assistants they still need to let the licensing inspector in when the provider is not home. Standard #:	22VAC40-111-100-A
Description:	Based on review of seven children's records not all had documentation of a physical within 30 of enrollment. Evidence: Child 1 with a start date of 9/4/17, Child 2, with a start date of 9/4/19, Child 3 with a start date of 9/20/19, and Child 5 with a start date of 9/12/18, did not have documentation of a physical.
Plan of Correction:	I will ask the parents for the missing information.
Standard #:	22VAC40-111-60-B
Description:	Based on review of seven children's records not all had the required information. Evidence: Child 4 with a start date of 2/25/19 did not have documentation of mother's work address or work phone number.
Plan of Correction:	Provider was able to up date the the child's record during the inspection.
Standard #:	22VAC40-111-140-B-1
Description:	Based on review of Assistant A's record and interviews, the assistant was caring for children and does not meet qualifications. Evidence: Assistant A was caring for children alone when LI arrived and does have a high school diploma or equivalent.
Plan of Correction:	The assistant will be a helper and not work alone.
Standard #:	22VAC40-111-180-A
Description:	Based on review of five house hold member' records not all had a T.B. test with in two years since the date of the most current T.B. test. Evidence: House Hold member 5'd T.B. test is dated 8/8/17.
Plan of Correction:	They will take a new test .
Standard #:	22VAC40-191-40-D-1-a
Description:	Based on review of five house hold member's records and one assistant's record not all had a Search of Central registry check with in 30 days. Evidence: House hold Member 1 living in home since 2015, and Assistant A Start date 7/10/19 did not have a search of central registry check.
