Comments:
An unannounced monitoring inspection was conducted on Thursday, October 17, 2019 to determine compliance with licensing standards. The inspector arrived at 10:00 PM and concluded the inspection at 1:50 PM. Today, there were eleven children, totaling 31 points, present with two caregivers. During the inspection, children were playing with playdough, reading books, singings, washing hands, eating lunch, and napping. Lunch consisted of: turkey meatballs, peas, peaches, wheat bread, and milk. The provider stated that medication is not administered at this time. Children?s equipment, learning materials, indoor space, and restroom, first aid kit and the nonmedical emergency supplies were inspected. The provider stated transportation is not provided at this time. Five children's records, two caregiver records, one household record, daily attendance records, fire drill, lockdown and shelter in place drill logs and all required postings were reviewed.

The provider was available for the inspection and was present at the exit interview at which time inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. Violations were cited and are documented on the violation notice.

Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observation and interview, the provider did not ensure that medications, including medications for caregivers and household members, were kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. Medication was observed in an unlocked cabinet over the counter in the kitchen.
2. Multiple prescription and nonprescription medications were observed in the unlocked cabinet.
3. The provider stated that she did not know that the medications need to be locked since they were in a cabinet out of reach.

Plan of Correction: "Will put a lock on upper cabinet where medication was."

Standard #: 22VAC40-111-90-A
Description: Based on a review of children's records, the provider did not obtain documentation of immunizations prior to a child attending the family day home.

Evidence:
The most recent immunization record for child #4 (20 months of age) was 8/31/2018. Child #4 was enrolled 5/07/2018.

Standard #: 22VAC40-111-90-C
Description: Based on a review of children's records, the provider did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
1. The record for child #3 (10 months of age) with an enrollment date of 2/11//2019, contained an immunization record dated 2/14/2019.
2. The record for child #4 (20 months of age) with an enrollment date of 5/7/2018, contained an immunization record dated 8/31/2018.

Plan of Correction: "Will ask parents to provide drs record of visit every 6 months for children under 2."

Standard #: 22VAC40-111-350-E
Description: Based on observation, the provider did not ensure that hot water at taps available to children shall be maintained within a range of 105?F to 120?F.

Evidence:
The hot water temperature in the children's bathroom was obtained using a digital thermometer. The hot water temperature registered at 147.6 degrees Fahrenheit. The provider attempted to adjust the water temperature during the inspection.

Plan of Correction: "was unaware of temp in bathroom for hot water. Was adjusted onsite"
