Technical Assistance:
Discussed the training due in March for Subsidy providers.
Discussed the immunization and physical requirements for children.

Comments:
An unannounced monitoring inspection was conducted today beginning at 9:30amwith the provider on site. There were 2 children (4 points) with the provider. A sample of 1 children's files and 2 staff/household member's files were reviewed. The children were engaged in the following: toy play, snack and hand washing. Interactions between the children and caregiver were positive. The physical space, training certificates, evacuation drills, attendance records and the Emergency Preparedness Plan were reviewed. The provider is current in MAT training, CPR and First Aid Training. Areas of non-compliance are identified in the violation notice. Description: Based on review of a child's record, the provider did not maintain documentation of the review of the child's information yearly.
Evidence:
1. Child #1 had a yearly review of the child's information documented on 9/21/2018 and needs an annual update.

Plan of Correction: I will ask the parent to update and return.

Standard #: 22VAC40-111-320-B
Description: Based on observation, the provider failed to cover all unused electrical outlets accessible to children under five years of age.
Evidence:
1. Two unused outlets in the family room to the left of the door near the stairs were not covered with protective covers.

Plan of Correction: I covered the outlets during the inspection.
