Comments:
An unannounced monitoring inspection was conducted between the hours of 10:10 am-12:15 pm. At the time of entrance, 3 children were in care with the provider present. Children were observed playing with toys, building with blocks, identifying colors, and singing and dancing during music time. A selection of 2 caregiver/household member records and 3 children records, the physical space, emergency supplies, evacuation drills, attendance records, and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. A Subsidy Health and Safety Inspection (SHSI) was also conducted this date. Standard #: 22VAC40-111-100-A
Description: Based on record review, the provider did not obtain documentation of a physical examination by or under the direction of a physician within 30 days after the first day of attendance.

Evidence:
Child #3 began attending the family day home on 8/13/19. There is no documentation of a physical examination in the record for Child #3.

Plan of Correction: I will give the form to the parent.

Standard #: 22VAC40-111-60-B
Description: Based on record review, 1 of 3 children's records did not contain all required information.

Evidence:
The record for Child #1 did not contain names of persons other than the custodial parents who are authorized to pick up the child

Plan of Correction: I will ask the parent to fill in the information.
