Violations:
Standard #: 22VAC40-111-70-A
Description: Based on record review, before the child's first day of attendance parents were not provided in writing all required information.

Evidence: Child 1's guardians were not provided information regarding liability insurance.

Plan of Correction: Parents will be provided information and sign the document.

Standard #: 22VAC40-111-280
Description: Based on observation, cleaning materials were stored in an area accessible to children.

Evidence: Disinfecting wipes were stored in a medicine cabinet accessible to children in the bathroom.

Plan of Correction: The wipes were stored on an upper shelf in the closet in the bathroom.

Standard #: 22VAC40-111-690-D
Description: Based on observation, the diapering surface did not meet all requirements.

Evidence:
1) The diapering surface was covered with a cloth covering. This covering cannot be cleaned and sanitized.
2) The diaper mat underneath the cloth covering was ripped and therefore cannot be cleaned and sanitized.
3) The diaper changing station was not cleaned and sanitized after a child was changed. After a child was changed, caregiver 1 was observed using a disinfecting wipe to clean and sanitize the surface.

Plan of Correction: The provider removed the cloth covering. The mat will be repaired to be able to be cleaned and sanitized. We will clean and sanitize the diaper changing station after every diaper change.
