Comments:
An unannounced renewal inspection was completed on 01/06/2020 from 1:45 P.M-4:00 P.M. At the time of the inspection there were seven children and two caregivers and one visitor present. There was a total of twenty-two points. The records for five children, two caregivers, two household members, emergency plan, first aid kit and emergency supplies, evacuation drills, chimney inspection, and required posted information were reviewed. The children were observed during naptime, transition to playtime, free choice activities, storytime, and counting activities. Exceptions to compliance are noted on the violation notice.

Description: Based on observation, the provider failed to ensure that a nonabsorbent surface was used for diapering.

Evidence:
1. The diaper changing mat that was on the diaper changing table had a tear in the middle of the mat, approximately one inch long, exposing the foam.
2. The provider verified that the diaper changing mat had a rip approximately an inch long in the middle of the mat.

Plan of Correction: The provider will purchase a new diaper changing mat.
