Technical Assistance:
We discussed relabeling toothbrushes and toothpaste for proper identification.

Comments:
An unannounced monitoring inspection was conducted today between the hours of 8:30 am through 10:30. There were a total of 9 children (-26- points) observed with 2 caregivers. A total of 5 children's files and 3 staff/household member's files were reviewed. The children were engaged in the following: toy play, games, breakfast, bathroom breaks, diapering, hand washing, and outside play. Areas of non-compliance are identified in the violation notice. Violations:
Standard #: 22VAC40-111-60-B
Description: Based on child record review, a child's record did not contain the results of a health examination.
Evidence: Child #1,first day of attendance, 12/17/18, does not have documentation of a physical examination in the record.

Plan of Correction: Caregiver spoke to parent and parent stated that she will provide the documents by end of the week.

Standard #: 22VAC40-111-680-B
Description: Based on observation, caregiver did not ensure that children's hands were washed with liquid soap and warm running water after toileting or diapering.
Evidence: Staff #1 was observed diapering a child. The child's hands were not washed after diapering. The child was returned to play.

Plan of Correction: Provider will speak with caregiver about proper hand washing.

Standard #: 63.2-1721.1-A
Description: Based on caregiver and household record review, applicant has not maintained repeat background checks every five years.
Evidence: The following caregiver and household member have not updated their background check five years from initial date.
