Please vary the times of your emergency drills; some drills should occur in the morning and others in the afternoon, representing all hours your program is operating.

Comments:
An unannounced monitoring inspection was conducted from 1:00 - 2:30 pm with the provider. There were two children in care, both 19 months of age, for a total of six points. The children were observed napping, and having free play with a variety of age-appropriate toys. Two children's records were reviewed. Three provider/household member records were reviewed and in compliance. Required information was posted. The emergency drill log was observed and in compliance. The provider is not Medication Administration Training (MAT) certified and cannot administer medication to children in care. Violations:
Standard #: 22VAC40-111-900-A
Description: Based on observation, a child was allowed to drink while lying down. Evidence: an empty bottle was observed in a crib with child #1 (19 months old) who was sleeping.

Plan of Correction: Will not give a bottle to a sleeping child.

Standard #: 22VAC40-111-100-A
Description: Based on a review of two children's records, the provider did not obtain documentation that each child has received a physical examination by or under the direction of a physician prior to the child's first attendance or within one month after attendance. Evidence: the record for child #2 (date of first attendance: 8/6/19) did not have documentation of a physical examination.

Plan of Correction: Will ask the parent to provide the missing documentation.

Standard #: 22VAC40-111-80-A
Description: Based on review of two children's records, the provider failed to have documentation of proof of the child's identity and age within 7 days of enrollment. Evidence: the record for child #2 (date of first attendance: 8/6/19) did not have documentation of proof of identity and age (proof of birth.)

Plan of Correction: The parent requested the document in August and is still waiting to receive it.
