Technical Assistance:

- Caregiver requirements discussed
-The ages of the children should be taken into account as to placement of disinfectants for diapering.

Comments:
An unannounced monitoring inspection was conducted on 01/08/2020 from 12:10 pm - 1:25 pm. At the time of entrance there were three children in care with the provider. The ages of the children counted for a total nine points. Children were observed napping and playing with toys. The provider was observed feeding a bottle to an infant in care.
Records were reviewed for three children, the provider and two household members.
The violations are listed on the violation notice and were reviewed with the provider at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-111-730-D
Description: Based on observation, the provider failed to ensure that medication, including medications for household members, shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. An inhaler, belonging to a household member, was observed placed on a dresser in a bedroom.
a. Child 1 was in this room when the inspector arrived at 12:10 pm.

Plan of Correction: The provider stated that this bedroom will no longer be used for care.

Standard #: 22VAC40-111-770
Description: Based on interview, the provider to ensure that there was a weather band radio available.

Evidence:
The provider stated that she did not have a weather band radio.

Plan of Correction: The provider stated that she had ordered a weather band radio and expected it to arrive soon.

Standard #: 22VAC40-111-250-A
Description: Based on observation, the provider failed to ensure that hanging items such as appliance cords shall be out of reach of children under five years of age.

Evidence:
1. Child 1, age 7 months, was placed in a car seat beside approximately six dangling cords that were hanging down from a bedroom dresser.
a. These cords were attached to a television, two cable boxes and a cell phone charger.
b. These cords were within reach of the car seat.

Plan of Correction: The provider stated that children will no longer be placed in this room for care.

Standard #: 22VAC40-191-40-D-4-a
Description: Based on record review, the provider failed to ensure that household members shall complete a sworn statement or affirmation when the individual begins residing in the home.

Evidence:
1. There was no sworn statement or affirmation on file for household member 1 who has resided in the home since October 2019.
2. The provider confirmed that household member 1 had not completed a sworn statement or affirmation.

Plan of Correction: Household member 1 will complete a sworn statement or affirmation today.

Standard #: 22VAC40-80-120-E-2
Description: Based on observation, the provider failed to ensure that the findings of the most recent inspection of the facility were posted on the premises of the family day home.

Evidence:
1. The findings from the most recent licensing inspection conducted on 10/03/2019 were not posted.
a. The provider had posted the findings from the subsidy health and safety inspection conducted on 10/03/2019.

Plan of Correction: The current findings were posted during the inspection.

Standard #: 63.2-1720.1-B-3
Description: Based on record review and interview, the provider failed to obtain a copy of the results of a search of the central registry maintained pursuant to ? 63.2-1515 and any child abuse and neglect registry or equivalent registry maintained by any other state in which the individual has resided in the preceding five years for any founded complaint of child abuse or neglect against him.

Evidence:
1. There is not a search of the central registry from the State of North Carolina for household member 1.
2. The provider indicated that household member 1 had resided in the State of North Carolina within the past five years.

Plan of Correction: A CPS check from the State of North Carolina will be requested within 10 days.
