Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and the upcoming renewal process to be conducted prior to the expiration of the current license on 06/02/2020.

Comments:
The following inspection took place between the hours of 1 pm through 3;30 pm. Today, I met with the Provider on site at the Family Day Home (FDH) located in Fairfax, Virginia. At the start of this unannounced monitoring inspection there were a total of 8 children observed (17 points) with 2 caregivers. A total of 8 children?s files, 2 staff files, and 2 household member?s files were reviewed. The children were engaged in the following: toy play, circle time, bathroom break, diapering, hand washing, hand washing, snack time, movie time, freeplay, and other organized activities. Violations:
Standard #: 22VAC40-111-100-A
Description: Based on review, 3 of 8 children files reviewed indicated that the Provider did not obtain documentation of a physical examination (by or under the direction of a physician) within 30 days of the child's first day of attendance.

Evidence -
1. On the date of inspection (01/06/2020), the following children were without documentation of a physical examination: Child #1, Child #2, and Child #6.
2. The start date for these children were listed as: 02/19/2019 (for Child #1), 10/21/2018 (for Child #2), and 08/27/2018 (for Child #6).

Plan of Correction: Documentation of a physical examination shall be obtained for all applicable children and placed on file.

Standard #: 22VAC40-111-90-C
Description: Based on review, 1 of 8 children files reviewed indicated that the Family Day Home did not obtain documentation of additional immunizations for a child (who was not exempt from the immunization requirements according to subsection B of this section) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection (01/06/2020), the most current immunization documentation on file for Child #1 was dated - 05/06/2019.
2. Based on information on file, on the date of inspection, Child #1 was under the age of two years.

Plan of Correction: The parent of Child #1 shall be contacted and an updated immunization page shall be obtained.

Standard #: 22VAC40-191-40-D-5-a
Description: Based on review, 1 of 2 household members files indicated that a Child Protective Services central registry check (CPS check) was not conducted within 30 days of a person in the home becoming 14 years old.

Evidence -
1. On the date of inspection (01/06/2020), a CPS check was not obtained for Household Member #1.
2. The Provider stated (during an interview conducted on the date of inspection) that Household Member #1 has now turned 14 years of age.

Plan of Correction: A request of a search of the central registry shall be obtained for Household Member #1. Once obtained, this documentation shall go on file.
