Discussed the use of bleach and water spray for changing tables and other surfaces. Recommended the state approved attendance form for use.
An unannounced monitoring inspection was conducted today between the hours of 8:30 am through 10:15. There were a total of 6 children (15 points) observed with 2 caregivers. A total of 3 children's files and 5 staff/household member's files were reviewed. The children were engaged in the following: toy play, ball play, trucks, coloring,and snack. Standard #: 22VAC40-111-100-A Description: Based on children's record review, the provider did not obtain a physical examination within 30 days of attendance.
Evidence: Child #1 and Child #2, first day of attendance, 12/02/2019, does not have documentation of physical examination in the record. Child #3, first day of attendance, 07/29/2019, does not have documentation of physical examination in the record.
Plan of Correction: Provider will ask the parents for the physical examination records.
Standard #: 22VAC40-111-60-B Description: Based on children's record review, children's record does not contain the required information.
Evidence: Child #1 and Child #2 first day of attendance, 12/2/2019, does not have a 2nd emergency contact, name address, and phone number.
Child #3, date of first day attendance, 07/29/2019, does not have medical insurance policy number in the record.
Plan of Correction: I will get the information from the parents today, Friday, January 10, 2020.
