Technical Assistance:
During inspection preparation for nap time to meet the needs of each child was discussed. Provider is to explore quite activities such as coloring books etc.

Comments:
An unannounced monitoring/renewal inspection was conducted today from approximately 12:30pm-2:30pm. There were 7 children directly supervised by 2 staff. The home, 2 staff records, 4 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. Per provider- There were no medications on site today. Children were observed preparing to rest and engaging in activities. . Diapering procedures were also observed. There was an adequate supply of books, toys, and materials for the children.

Description: Based on observation and conversation with provider it was determined that the vehicle used for transportation of children in care was not licensed according to state law.

Evidence: During inspection provider stated, that the vehicle on site was used for transportation and upon observation the registration for said vehicle was expired as of June 2019

Plan of Correction: Already fixed

Standard #: 22VAC40-111-60-A
Description: Based on review of Child records and interview with provider it was determined that all required information was not maintained.

Evidence: Child 1 was missing health insurance policy number Child 3 was missing first day of attendance, and actions to take if exposed to listed allergen Child 4 missing health insurance policy number.

Plan of Correction: Information will be collected from parents.

Standard #: 22VAC40-111-640-4
Description: Based on Observation it was determined that Child A was being forced to nap.

Evidence: During inspection, Child A was on their mat crying, and actively trying to get up and was prevented by caregiver A; Caregiver A had their arm and upper body over the child, resting on their mat.

Plan of Correction: A behavioral plan will be written with the mother to establish the procedure to help the child learn to follow a routine and to participate properly.
