Technical Assistance:
The provider continues to use one crib for two different children who have different sleep schedules. This routine is permitted because the provider reports that the sheets are changed and the mattress is sanitized in between each child's use.

Comments:
An unannounced monitoring inspection was conducted today. The provider and two assistant caregivers were caring for 7 children, totaling 24 points. Children's records, the physical plant, children's equipment, children's nutrition, administrative records, background checks, emergency preparedness items, and caregiver:child interactions were inspected today. It was reported to the Inspector that no children's medications were on site today.

The children were observed eating morning snack, assembling puzzles, playing with toys, listening to music, taking their morning nap, and engaging in interactions with each caregiver.

Violations:
Standard #: 22VAC40-111-960-B
Description: Based on observations, it was determined that two infants who could not hold their own bottles were not held when being fed.
Evidence:
The Inspector observed two infants seated in bouncy seats when being fed their bottles. Neither of these children could hold their own bottle. Both the provider and the assistant caregiver sat in chairs next to the seats and held the bottles for each infant. Neither caregiver held a child while feeding their bottles.

Plan of Correction: Both infants were picked up and held immediately during the inspection.

Standard #: 22VAC40-111-960-E
Description: Based on observations, it was determined that infants bottle were not labeled with each child's name.
Evidence:
The bottles for two infants were not labeled with each child's name.

Plan of Correction: The name labels have worn off. I will label the bottles again.

Standard #: 22VAC40-111-200-A
Description: Based on a review of caregiver training documents, it was determined that the provider did not conduct orientation training for one assistant caregiver before the end of her first week of assuming job responsibilities.
Evidence:
Assistant caregiver B started employment on 8/1/19. The provider has not completed orientation training for her.

Plan of Correction: We will do the training this week.

Standard #: 22VAC40-111-240-A
Description: Based on observations, it was determined that one area of the home was not maintained in an adequately safe condition. In addition, one piece of equipment has not been maintained in operable condition.
Evidence:
1 - Upon arrival, the Inspector observed one lit candle burning on the counter in the kitchen area that is directly adjacent to the child care area. This is a safety hazard in the event that the candle were to be left unattended.
2 - The diaper trash can is not working as required. This trash can does not open when operated by foot because the lid has broken.

Plan of Correction: 1 - The candle was immediately blown out during the inspection.
2 - The trash can broke on Friday. I will get a new trash can.

Standard #: 22VAC40-111-520-A
Description: Based on observations, it was determined that one infant under the age of 12 months was not provided a crib for sleeping.
Evidence:
The Inspector observed one 6 month old infant placed on the bed of a household member. This infant was not placed in her crib for her nap time.

Plan of Correction: The child doesn't sleep in the crib, so the parents requested we let her sleep on the bed. We still stop letting her sleep on the bed and always put her down in her crib for nap.

Standard #: 22VAC40-111-530-F
Description: Based on observations, it was determined that soft bedding was used by infants while asleep in their cribs.
Evidence:
Upon arrival, the Inspector observed two infants (both age 8 months) asleep in their cribs with soft blankets on top of them.

Plan of Correction: The blankets were immediately removed during the inspection. We will only use the sleep sacks for the infants.
